New patients need to plan on arriving at least 15 minutes prior to your appointment to fill out paperwork, or you can download new patient forms by clicking the link below and bring them with you to your appointment. Please bring all insurance cards and a list of your current medications with you as well. If your insurance plan requires a referral, please have your primary care physician submit the referral prior to your appointment.
Returning patients also need to plan to arrive 15 minutes early to fill out paperwork, if this is your first visit of the year. Paperwork is updated once a year and filled out at the first appointment of a calendar year beginning January 1st. It is also very important to bring all insurance cards, a list of your current medications, and any referrals if needed.
To evaluate the health of the back of your eye, you will be dilated for your exam. Whether you feel comfortable driving after your exam is a personal choice. Many people feel okay to drive after dilation; others feel more comfortable having a friend or family member drive them home after their appointment. We provide complimentary sunshades for you to put on after your appointment to help reduce the uncomfortable glare of the sun. If you think you will be unable to drive after your appointment, please plan on having a friend or family member accompany you to your exam.
